Damon, my Brother from another mother! You can do this! I know, because I have been there. And I have been clean for sixteen years; ONLY by the grace of Jesus.
You have nothing to fear, you have nothing to lose. Call on Him. He knows everything anyway. Just go to Him and ask Him to forgive you. There is nothing that you may have done that He will not forgive if you go to Him sincerely. Jesus says in the Bible; "Their sins and lawless acts I will remember no more." Hebrews 10:17
He will forgive you and strengthen you through this process. I have been and will continue to be praying for you!
There will be a day when we will all go out to dinner and celebrate and laugh; offering praise and gratitude to our Lord for your new life!!!

Damon, I am praying for you brother. I myself have also been through some tough situations. Coming out of those hard times...I learned this:
There is nothing you can do to make God love you less. He loves you more than you can even know. He forgives and He heals.
May you be comforted and loved as you continue through this process.
